Wiki history of present illness

i have a provider that documents like this:

hpi:
this 84 year old male presents with:
1. chronic conditions
HTN, ELEBATED PSA, GERD, CRI

this is all that is put in hpi: can i count this: or does each chronic illness have to be addressed: htn, elevated blood pressure...etc..


please advise
