Patient is referred to a new provider to find out if he has restrictions from a metacarpal fracture. The fracture is healed as stated by the new physician. Would you code the fracture with an A for new physician initial visit or with the Z09?

The A is not for new physician. It is for when the patient is receiving active treatment of an active injury. If the condition no longer exists and requires no treatment you use the follow up cod Z09.
