Please advise:

Our office is strictly Pediatrics. We require parents or legal guardians present to the office in person to pick up ADHD prescriptions/refills on a monthly basis. The nurse will then ask the parent/guardian a couple questions pertaining to how the pt is responding to the Rx.

We have always billed 99211 for this service. In most cases, the pt does not present with the parent or guardian, so there is no face to face between the nurse and the pt. CPT code 99211 does not state that face to face with the patient is required, therefore we can bill 99211 for this purpose, correct???

This is not appropriate use of E&M codes for prescription refills. Novitas-solutions has a great list of Frequently asked E&M questions and this is addressed and the Medicare specified guideline is shared on the last question.

The patient must be evaluated to bill for a 99211. Medicare requires a problem/complaint by the patient and face to face encounter.
