Wiki Expanded Endoscopic Skull Base surgery

Is anyone out there doing this procedure where Neurosurgery and ENT are working together? It's similiar to the open skull base codes but this is endoscopic. We've been billing an unlisted 64999 but it's causing all kinds of trouble with carriers. I hoping to find out how others are handling this?
