Wiki Excision depth determined by anatomic descriptors?

New to this and just need a second opinion...

Can one determine excision depth (of a benign mass) based on the anatomical description given in the Op note?

For example. Starts out saying dissection into subcutaneous but then told after it was coded that because a digital nerve was excised, it should have been coded deeper because that nerve is below the cleland ligament and that ligament is in the fascia. So it should have been coded as a subfascial excision.

I know anatomy but that's pretty over my head and I was taught never to assume something as it may not be what it appears.
