Wiki DX for vaccination visit

Hello,
The pediatrician checks up on a patient that comes in to receive his/her vaccinations.
Pediatrician wants to codes 99211 or 99212 but does not have a valid DX.
What would be a valid DX to use for this visit???

unless the patient has a medical condition that warrants a different diagnosis, you would use the vaccination dx for the E&M code. Usually if the only reason for the visit is vaccinations, it is not recommended that an E&M code be added to the visit.
 
if the md did vaccine counseling, that would be added. otherwise the md should not waste his time seeing a shot only patient. lburke cpedc
 
If a physician took the time to review the prior immunization records and documented in the chart that they spent time doing so.. would that support a 99211?
