We have a patient whose general anesthesia case was discontinued post admin of anesthesia due to cardiac event. The patient later came back same day for the case under local. How would we bill the secondary completed case. Would this be considered repeat (76), unplanned return to OR (78), or staged/planned procedure (58). I question if its really a repeat procedure given the first procedure was not fully completed. I can rationalize either 58 or 78. My understanding is modifier 53 does not affect global days, so same day would be within the global period. It was technically related to the first case and planned after the first was discontinued (58), however it could also be considered an unplanned return to the OR as it wasn't the plan prior to the first procedure (78). Any thoughts on this?
