CPT code 21400; Closed treatment of fracture of orbit, except blowout; without manipulation. (Closed treatment of an orbital fracture without manipulation. This means that a healthcare provider treats a fracture around the eye socket without needing to physically adjust or move the bones back into place.)
My question is, "What are the documentation requirements to bill CPT code 21400in any setting?"
The provider wrote a complex ED hospital consult with reason, history of present illness, ROS, radiology findings, physical exam/Focused HEENT, Imaging/CT findings "personally review", impression and assessment plan, recommendation.
I would bill only the 21400 and not the hospital consultation OR bill both together?
