Alicia, for left heart catheterization, w/o PCI, CPT code 93458 is used which includes catheter placement in one or more coronary arteries for coronary angiography. "This procedure may also include intraprocedural injections for coronary angiography, imaging interpretation and supervision, and left heart catheterization." As per BCBS pertinent claims denials for heart catheterization, there is additional criteria should be met. If the denial MN - N50 CARC code - make sure indication and co-morbidities are listed on the claim as supportive DXs for CPT 93458 procedure, plus abnormal EKG LCD valid DX.
