I have a patient that went in to hospital for observation and also an NST. I billed as a 99219 and 59025-26. I received a denial for the 99219 as a bundled code with the 59025-26. If the Dr did and observation and the NST prof component how should this be billed?

If the documentation supports a separate E&M service from the test, then a modifier 25 is required for 99219.
