On occasion, patients come into the office to have casting removed and replaced, (too loose, got wet, etc) when the provider is outside the office performing surgery. I don't believe this service is billable as cast application or as a 99211 since the provider is not in the office suite. However, I need clarification.

My understanding is incident to requires the provider be in the office suite in order to bills these as 99211. However, I have a provider questioning me as his "other office" bills nurse only visits all the time.

Check your MAC on this. This can vary from MAC to MAC and even state rules differ from MACs as well.

I will agree with you though. Your physician cannot bill anything "incident to" if they are not available. So if the provider is in surgery, they would not be available for questions.

Just because another office staff will bill incident to when the provider is in surgery is no reason for you to do so.
