Received a denial from medicaid stating an injection and ultra sound was included in the Ob visit. Can we put a 25 on the 59425? We never have before.

Wiki can you use modifier 25 with 59425 when an inj. and u/s was done?

No, you cannot. Modifier 25 is to only be used on a E/M service.
