Patient sustained bilateral patellar fractures and bimalleolar fracture. Dr. did ORIF on LT patella, fracture care on RT patella and plans to take patient back to surgery in 3-4 weeks for ORIF of bimalleolar fracture after swelling subsides. My question is should we charge fracture care on bimalleolar fracture initially and then use modifier -58 on ORIF bimalleolar fracture 4 weeks from now.
