Can anyone tell me if 58740 is reimbursed as well as procedure 58140? Or do you just bill 58140?

I would use the 58140 and add the 22 modifier if the surgeon documented the additional time to take down the adhesions. Should be 45 mins or greater. Normaly the problem is the physician not doing the documentation.
