The provider accesses the middle cranial fossa using an infratemporal (below the temporal fossa) preauricular (in front of the external ear) approach, sometimes called the transparotid approach, to expose a lesion or defect at the base of the skull. The approach can also include a mastoidectomy, resection of the sigmoid sinus and/or decompression/mobilization of the auditory canal or petrous carotid artery. The selection of a particular approach will depend on the location and size of the lesion.
