You Be the Coder:

Same-Day Injections

Test your coding knowledge.  Determine how you would code this situation before looking at the box below for the answer.   Question: When a patient has gentamicin and an injection of Zoladex the same day, how should I bill?

Rhode Island Subscriber

Answer: Code for both. For the gentamicin, use 90788 (Intramuscular injection of antibiotic [specify]) and J1580 (Injection, Garamycin, gentamicin, up to 80 mg). Report 96400 (Chemotherapy administration, subcutaneous or intramuscular, with or without local anesthesia) and J9202 (Goserelin acetate implant, per 3.6 mg) for the Zoladex. Bill an office visit (99211-99215) also.

For Medicare, bill only the chemotherapy administration and the drugs. Code 90788 will be denied as included in the other services.
