Robotic Mullerian Cyst Excision

Question: I have a question about coding a robotic excision of Mullerian duct cyst remnant and partial cystectomy.  While excising the Mullerian cyst the doctor realized it had grown through the bladder wall in the area of the trigone. A partial cystectomy was required to completely remove the cyst. How should I code this?

Answer: There is no exact CPT® code to use for this procedure. Report 51999 (Unlisted laparoscopy procedure, bladder) for the laparoscopic partial cystectomy and removal of the Mullerian duct cyst. Benchmark the procedure to 51550 (Cystectomy, partial; simple) and 55680 (Excision of Mullerian duct cyst). 
 
You will need to send a detailed operative report and a covering letter on what your urologist did and why, as well as why he used robotics. Include the fee you expect to be paid, making sure to ask for at least 50 percent more than the fees for the above bench marked procedures.
