Determine Exact Testosterone Preparation Before Coding

Question:  How do I code 400 mg of testosterone?

Answer:
The answer depends on which testosterone preparation your urologist used. 


The first type is testosterone enanthate. If your urologist gave this preparation, you will report J3130 (
Injection, testosterone enanthate, up to 200 mg). Because the code specifies 200 mg, you’ll report two units in box 24G (days or units) of the CMS-1500 form.

The second preparation your urologist may use is testosterone propionate. For this type, report J3150 (Injection, testosterone propionate, up to 100 mg). Because this code specifies 100 mg, you’ll report four units in box 24G of the CMS-1500 form .
