Correctly Report Martius Flap Procedure

Question: The urologist completed a Martius flap procedure during the same encounter as a urethrovaginal fistula closure. How should we code these?

Answer: The correct code is 57311 (Closure of urethrovaginal fistula; with bulbocavernosus transplant). This code includes the repair of the urethrovaginal fistula and a Martius bulbocavernosus transplant flap for the repair.

Also note: If the urologist completed repair of a vesicovaginal fistula with a Martius bulbocavernosus flap, submit 57320 (Closure of vesicovaginal fistula; vaginal approach) for the vesicovaginal repair and 57311-52 (Reduced services) for the harvesting and use of the Martius flap without a urethrovaginal repair.
