Question: Our doctor performed fulguration on 28 genital warts comprising about 14 sq cm. Is CPT 54055 or CPT 54065 the correct code?

Answer: Use CPT Code 54065 (Destruction of lesion[s], penis [e.g., condyloma, papilloma, molluscum contagiosum, herpetic vesicle], extensive [e.g., laser surgery, electrosurgery, cryosurgery, chemosurgery]). The code includes every method for removal, and 14 sq cm would certainly be considered extensive. Although 54055 (Destruction of lesion[s], penis, simple; electrodesiccation) describes the fulguration procedure specifically, it does not adequately describe the work involved in destroying 28 genital lesions.
