Massachusetts Subscriber Answer: You should report 54640 (Orchiopexy, inguinal approach, with or without hernia repair) for a bilateral orchiopexy, inquinal approach. Append modifier 50 (Bilateral procedure) to 54640 to indicate that the urologist performed the procedure bilaterally. This procedure code includes the dissection, removal, and detachment of the cremasteric muscles, so you do not need another code.
