Reader Questions:

Select Unlisted for Mitrofanoff Channel Dilation

Question: My doctor performed a cystoscopic examination via a Mitrofanoff cutaneous stoma, and using a snare removed several small calculi from the bladder. I know how to code that procedure, but he also dilated the Mitrofanoff channel, and I cannot find a code for that. How should I report that portion of the procedure?Utah SubscriberAnswer: There is no specific CPT code for the dilation of the cutaneous Mitrofanoff stoma. You should therefore report that portion of the procedure using an unlisted code from the intestinal section, such as 44799 (Unlisted procedure, intestine). For the instrumental removal of multiple bladder stones, report 52315 (Cystourethroscopy, with removal ...of calculus... from bladder; complicated). The diagnoses you'll report will likely be 594.1 (Other calculus in bladder) for the bladder calculi and 997.5 (Urinary complications) for the cutaneous or stomal stenosis.
