Question: My urologist performed surgery for an exchange of a double-J stent. This patient was a male with an ileal conduit. He was missing a kidney on the right side. My urologist performed an ileoscopy. Can you help me? I cannot find a CPT® code for this. AAPC Forum Subscriber
Answer: Since an intestinal pouch or ileal conduit represents a substitute bladder, code 52000 (Cystourethroscopy (separate procedure)) is recommended in the 2021 CPT® manual for a pouchoscopy or endoscopy (ileoscopy) of an ileal conduit. If the change of the stent was via a retrograde endoscopic approach within the ileal conduit, bill 52332 (Cystourethroscopy, with insertion of indwelling ureteral stent (eg, Gibbons or double-J type)) for the stent exchange. You could also report 50688 (Change of ureterostomy tube or externally accessible ureteral stent via ileal conduit) for the external coaxial change of a JJ stent via an ileal conduit. If your urologist uses imaging guidance for the 50688 service, you can also report code 75984 (Change of percutaneous tube or drainage catheter with contrast monitoring (eg, genitourinary system, abscess), radiological supervision and interpretation), per CPT®.
