Rely on Testicular Torsion Option

Question: A patient came in with testicular torsion of the left testicle. My physician reduced it, and then as a precaution performed the same procedure on the right side. I know that CPT® states that the contralateral side is included and cannot be separately billed, but this code can be billed bilaterally. Is the bilateral only billable when the other side also has a torsion?

Answer: If the procedure was surgical, you should report code 54600 (Reduction of torsion of testis, surgical, with or without fixation of contralateral testis), which does include fixation of the contralateral testis. You can bill this code bilaterally if both testes were twisted and required reduction.
