Focus on Correct Option for Bladder Neck Incision

Question: Would an incision of a bladder neck contracture be coded as 52276 or 52214? According to the op note, “After cystoscopy, a 1000 micron holmium fiber was introduced, and the bladder neck contracture was incised.”

Answer: The best code for a laser incision of the bladder neck would be 52214 (Cystourethroscopy, with fulguration (including cryosurgery or laser surgery) of trigone, bladder neck, prostatic fossa, urethra, or periurethral glands). You should report N32.0 (Bladder-neck obstruction) as the ICD-10-CM code on this claim.

Don’t miss: “Code 52214 is used to report cystourethroscopy performed to stop bleeding by fulguration of the trigone, bladder neck, prostatic fossa, urethra, or periurethral glands,” according to CPT® Assistant Vol. 26 No. 5. “This code should be reported only once per operative session. It is not appropriate to report code 52214 multiple times even if several areas were fulgurated.”
