Figure Out These Stone Procedures

Question: My physician insists that you can charge 52356 for lasering stone in the right kidney and 52352 for basketing a separate ipsilateral right ureter stone. Are there any bundling issues with these two codes?

Answer: You can report both codes on your claim. You should report the following for your clinical scenario:
- 52356 (Cystourethroscopy, with ureteroscopy and/ or pyeloscopy; with lithotripsy including insertion of indwelling ureteral stent (eg, Gibbons or double-J type))-RT (Right side) for the renal stone. The ICD-10-CM code is N20.0 (Calculus of kidney).
- 52352 (Cystourethroscopy, with ureteroscopy and/or pyeloscopy; with removal or manipulation of calculus (ureteral catheterization is included))-XS (Separate structure …)-RT for the stone in the ipsilateral ureter. The ICD-10-CM code is N20.1 (Calculus of ureter). The American Urological Association (AUA) and CPT® consider this as a stone in a separate structure, hence the use of modifier XS.
