Different Side Means Separate Lead Removal Coding

Question: My doctor took out an old interstim generator and lead on the patient's right side. He put in a temporary lead on the opposite side, and later put in another generator. Can I bill the removal?Florida SubscriberAnswer: Yes, you can bill for the removal. First, report 64561 (Percutaneous implantation of neurostimulator electrodes; sacral nerve [transforaminal placement]) for the placement of the temporary lead. Append modifier 59 (Distinct procedural service) and LT (Left side).Next, report 64585 (Revision or removal of peripheral neurostimulator electrodes) for the removal of the previously surgically inserted lead. Append both modifiers 51 (Multiple procedures) and RT (Right side). Finally, report 64595 (Revision or removal of peripheral or gastric neurostimulator pulse generator or receiver) with modifier 51 appended for the removal of the generator.
