Watch Whether Suprapubic Tube Opening Needs a Revision

Question: What CPT® code would be used when a patient is taken back to the OR after an initial suprapubic cystostomy for revision of the suprapubic stoma, ostium, and replacement of the suprapubic tube?

Answer: Generally, when theprovider changes a suprapubic tube, the appropriate codes are either 51705 (Change of cystostomy tube; simple) or 51710 (… complicated). The code for a complicated change of the cystostomy tube, 51710, would also include a revision of the stoma if also performed.

If you also are in the 90-day global period of the initial cystostomy (code 51040, Cystostomy with drainage), the case might also justify appending modifier 78 (Unplanned return to the operating/procedure room by the same physician or other qualified health care professional following initial procedure for a related procedure during the postoperative period). You’ll need to check the patient’s records to verify whether it qualifies for modifier 78.
