Reader Question:

Report the Most Accurate Penile Surgery Code

Question: What would we report for preputioplasty? This is a limited dorsal slit with transverse closure for a patient with phimosis. Would you use a circumcision code with a 52 or would this be an unlisted code?

Answer: Report either 54000 (Slitting of prepuce, dorsal or lateral [separate procedure]; newborn) or 54001 (... except newborn) based on the patient’s age.

Modifier 52 (Reduced services) would not be appropriate with a circumcision code in this case. Since a dorsal slit may be part of the circumcision procedure itself, and there is a CPT® code for a dorsal slit, you can code what your urologist actually performed by using the dorsal slit code and avoiding the use of modifier 52 on the circumcision code.
