Under ICD-9, you should report the condition as 233.4 (Carcinoma in situ of prostate), which crosswalks to D07.5 (Carcinoma in situ of prostate) in ICD-10.
You should report PIN I or PIN II using 602.3 (Dysplasia of prostate) in ICD-9, which crosswalks to N42.3 (Dysplasia of prostate) in ICD-10.
There are no specific ICD-9 or ICD-10 codes for prostatic atypical small acinar proliferation (ASAP), so use the “other specified” code. That’s 602.8 (Other specified disorders of prostate) in ICD-9, which crosswalks to N42.89 (Other specified disorders of prostate) in ICD-10.
