Lysis of Spermatic Nerve Adhesions Falls Under 64722

Question: The urologist performed scrotal exploration, lysis of adhesions of the right spermatic nerve, neurolysis, and division of cremasteric fibers. I know the scrotal exploration code is 55110 but is most likely going to be included with the main procedure. I cannot locate a code for the lysis of adhesions. Would code 54640 work for the above procedures listed?

Answer: No, 54640 (Orchiopexy, inguinal approach, with or without hernia repair) is not your best option as an orchiopexy was not performed, and because you actually can report these procedures with another specific CPT® code.

Having documented lysis of adhesions of the spermatic nerve, neurolysis, and division of cremasteric fibers, bill code 64722 (Decompression; unspecified nerve[s] [specify]). The work of a scrotal exploration, code 55110 (Scrotal exploration), is included in this operative procedure so is not separately billable.
