Question: Since 89321 has no RVU value and carries a zero value, how do I assign an RVU for this?

Answer: You are correct that 89321 (Semen analysis; sperm presence and motility of sperm, if performed) does not have any assigned relative value units (RVUs). However, it does have a fee assigned on the 2014 Clinical Diagnostic Laboratory Fee Schedule. The non-adjusted non-facility fee is about $17. Actual payment would depend on the state where the test is performed.
