Answer: How you code for the incision of a stricture at the ureteropelvic junction (UPJ) will depend on the surgical technology that is used.
For a percutaneous approach, submit 50575 (Renal endoscopy through nephrotomy or pyelotomy, with or without irrigation, instillation, or ureteropyelography, exclusive of radiologic service; with endopyelotomy [includes cystoscopy, ureteroscopy, dilation of ureter and ureteral pelvic junction, incision of ureteral pelvic junction and insertion of endopyelotomy stent]).
For a ureteroscopic approach, report 52345 (Cystourethroscopy with ureteroscopy, with treatment of ureteropelvic junction stricture [e.g., balloon dilation, laser, electrocautery, and incision]). Also bill for placement of a ureteral stent with 52332 (Cystourethroscopy, with insertion of indwelling ureteral stent [e.g., Gibbons or double- J type]). Include ICD-10 diagnostic code Q62.11 (Congenital occlusion of ureteropelvic junction).
