Know Your Code Choices for for interStim Battery Service

Question: What would the correct CPT® code be for replacing an interStim battery? The lead was not replaced.

Answer: Three codes represent interStim battery services, depending on exactly what service is performed. The choices are:

  • 64585 – Revision or removal of peripheral neurostimulator electrode array
  • 64590 – Insertion or replacement of peripheral or gastric neurostimulator pulse generator or receiver, direct or inductive coupling
  • 64595 – Revision or removal of peripheral or gastric neurostimulator pulse generator or receiver.

In the above case for replacement of the neurostimulator pulse generator, use code 64590. However, for accuracy, be sure to check your physician’s documentation to verify the procedure details so you can select the correct code. Also be sure to check the guidelines for each code to be sure you report it correctly.

Removal of the generator (code 64595) is included in the replacement of the generator (64590) and should not be separately billed.
