Question: What codes should we use for intravenous pyelography (IVP) and kidney, ureter, bladder x-ray (KUB) for the physician and the ambulatory surgical center where the services will be rendered?

Answer: For IVP and KUB procedures the physician would report 74400 (urography [pyelography], intravenous, with or without KUB, with or without tomography) with the modifier -26 (professional component) appended. Because the intravenous pyelography descriptor includes the words with or without KUB, it would be inappropriate to submit a separate code for this procedure.
