Urology Coding Alert

Reader Question:

Include Abdomen, Pelvis Codes for Spiral CT

Question: About six months ago we purchased our own CT scanner. We were informed that when performing a spiral CT to look for stones, we could bill for both abdomen and pelvis scans. Is this proper billing?

North Carolina Subscriber Answer: Not only can you report codes 72192 (Computed tomography, pelvis; without contrast material) and 74150 (Computed tomography, abdomen; without contrast material) for a spiral CT (sometimes called a spiral urogram), but you can also report CPT code 76375 (Coronal, sagittal, multiplanar, oblique, 3-dimensional and/or holographic reconstruction of computed tomography, magnetic resonance imaging or other tomographic modality).

According to the CPT Assistant, March 2002, page 11, "CPT code 76375 is intended to report studies that have been performed from one plane to another." Be sure the claim is justified, though - the radiology report must clearly show that you performed reconstruction. The words "reconstructive view" or "reconstruction" should appear in the documentation.
