Hernia Repair Coding Depends on Surgical Method

Question: One my doctors did a laparoscopic prostatectomy with robotic assistance and umbilical hernia repair revision. Are there any separate hernia revision codes besides the repair code 49585?

Answer: Take a look at 49652 (Laparoscopy, surgical, repair, ventral, umbilical, spigelian or epigastric hernia [includes mesh insertion, when performed]; reducible). With an open closure of an abdominal wound, the umbilical hernia repair would be included in the wound closure itself and should not be separately billed.

In your case, however, for a laparoscopic procedure and the finding and repair laparoscopically of an umbilical hernia, report 49652 along with the primary laparoscopic procedure performed.

In your case report the following:

·         55866 (Laparoscopy, surgical prostatectomy, retropubic radical, including nerve sparing, includes robotic assistance, when performed) for the laparoscopic/robotic prostatectomy, and

·         49652 for the laparoscopic/robotic umbilical hernia repair.
