Choose J9280 for Mitomycin Administration

Question: I have a physician who would like to give a patient Mitomycin. We normally use BCG but because of the national shortage he wants to use Mitomycin instead. I’m not familiar with compound drugs and am unsure of what HCPCS code I should use. What do you recommend?

Answer: The correct choice is J9280 (Injection, mitomycin, 5 mg).

Background: Mitomycin is an anticancer drug that providers often give in conjunction with other cancer medication(s). The standard intravesical dose is 40 mg. Therefore, bill J9280 with 8 units in the unit column 24G when administering 40 mg. of Mitomycin intravesically.

Take note that code J9280 represents only 5 mg of the drug, not the administration of the drug. You should bill that separately with CPT® code 51720 (Bladder instillation of anticarcinogenic agent (including retention time)).
