Approach Determines Correct Endopyelotomy Code

Question: My physician did a laser endopyelotomy of the ureter to allow for stent placement. This was all done through cystoscope. Is there a code for this procedure?

Answer: It sounds like your doctor actually did this ureteroscopically. If so, the correct code is likely 52345 (Cystourethroscopy with ureteroscopy; with treatment of ureteropelvic junction stricture [eg, balloon dilation, laser, electrocautery, and incision]). Otherwise, the proper code is likely 52342 (Cystourethroscopy; with treatment of ureteropelvic junction stricture [eg, balloon dilation, laser, electrocautery, and incision]). In present day urology, the first procedure (52345) is commonly performed while the latter (52342) is infrequently used.
