If you answer "No" to any of these questions, the services cannot be billed as incident-to. Instead, they should be reported using the NPP's PIN.

 1. Are the incident-to services incidental to the physician's professional services? [ ] yes [ ] no

 2. Did the physician initially see the patient and establish a plan for treatment? [ ] yes [ ] no

 3. If this is an established patient being seen for a new problem, has the physician examined the patient? [ ] yes [ ] no

 4. Are the services and supplies typically provided by this practice? [ ] yes [ ] no

 5. Were the services provided in the physician's office, the patient's home, or an institutional office setting (e.g.,   leased office space in a nursing home)? [ ] yes [ ] no

 6. Was the supervising physician physically present within the office's four walls while the services were rendered?
 [ ] yes [ ] no

 7. Are the physician and the NPP retained by a common employer?* [ ] yes [ ] no

 * Exception: Some PAs may serve as independent contractors and bill services incident-to.
