Due to a production error in the August 2005 issue, Urology Coding Alert inadvertently reran an article from the June 2005 issue. The article, headlined "Don't Hesitate to Ask Doctors to Clear Up Documentation" in June, appeared under the headline "Query Your Urologists to Clear Up Documentation Confusion" in August. We apologize for the repetition.
