Consider Coding Breakdown for Lumbar Puncture Cisternography

Question: What’s the CPT® coding for a fluoroscopic-guided lumbar puncture performed for a nuclear medicine (NM) cisternography?

Answer: You will report two codes for this service: one for the lumbar puncture and one for the cisternography.

For the lumbar puncture, you will report 62323 (Injection(s), of diagnostic or therapeutic substance(s) (eg, anesthetic, antispasmodic, opioid, steroid, other solution), not including neurolytic substances, including needle or catheter placement, interlaminar epidural or subarachnoid, lumbar or sacral (caudal); with imaging guidance (ie, fluoroscopy or CT)). As you can see in the code description, this includes fluoroscopic guidance, which you do not report separately.

For the cisternography, you will report 78630 (Cerebrospinal fluid flow, imaging (not including introduction of material); cisternography). Keep in mind that no National Correct Coding Initiative (NCCI) edits exist between the two reported codes.
