Hysterosonography and Hysterosalpingogram

Question: Our radiologists plan to begin performing hysterosonographies and hysterosalpingograms. Which codes should I use for these? Can I include ultrasonic guidance with the hysterosonography codes or is it included in the primary procedure? Nevada Subscriber Answer: When reporting a hysterosalpingogram, use 74740 (Hysterosalpingography, radiological supervision and interpretation) along with 58340* (Catheterization and introduction of saline or contrast material for hysterosonography or hysterosalpingography) to describe the injection of saline or contrast. The procedure code remains the same for hysterosonography (58340), but 76831 (Hysterosonography, with or without color flow Doppler) would be assigned in place of 74740. Generally, the color flow Doppler is used as the guidance modality for hysterosonography, so there is no reason to add an ultrasound guidance code.
