Following the exam code, you will use Z86.16 (Personal history of COVID-19) to document the medical necessity for the exam, which for this patient is a diagnosis and treatment for COVID-19. Excludes1 note: Parent code Z86.1- carries Excludes1 notes of “personal history of infectious diseases specific to a body system” and “sequelae of infectious and parasitic diseases.” Z86 Parent Code note: You will code any follow-up examination after treatment first with Z09 (Encounter for follow-up examination after completed treatment for conditions other than malignant neoplasm).
