Question: My podiatrist removed a ganglion cyst from the patient’s left foot. According to the medical documentation, after the patient was appropriately prepped and anesthetized, my podiatrist made an incision on the dorsal aspect of the foot anterior to the ankle joint. They then dissected down through the subcutaneous tissue, protecting the nerves and vessels. My podiatrist identified the lesion affecting the tendon sheath and removed the lesion. My podiatrist repaired the tendon sheath, irrigated the wound with antibiotic solution, and closed the wound in layers. Which CPT® code should I report for this procedure?

Answer: You should report 28090 (Excision of lesion, tendon, tendon sheath, or capsule (including synovectomy) (eg, cyst or ganglion); foot) on your claim. This procedure also includes a synovectomy, which is removing the inflamed or diseased synovium from the extensor tendon sheath of the foot to relieve pain.
