Differentiate Between Dislocation and Fracture Repair

Question: My podiatrist performed closed manipulation of a dislocated toe. My colleague thinks we should report 28515, but I don’t think that is the correct code. Can you help me?

Answer: You are correct. You should not report 28515 (Closed treatment of fracture, phalanx or phalanges, other than great toe; with manipulation, each) for a dislocation. Code 28515 is for a closed treatment of a fracture, per the code description.

For a dislocation, you can look to 28630 (Closed treatment of metatarsophalangeal joint dislocation; without anesthesia) if the dislocation is at the metatarsophalangeal (MTP) joint.

If your podiatrist performed closed treatment of an interphalangeal joint dislocation, you should look to 28660 (Closed treatment of interphalangeal joint dislocation; without anesthesia).
