: The answer to this question is it depends on the particular payor and their requirements. If an insurance payor tells you what they require in terms of documentation of time, follow their policy. If they do not tell you, you have at least two options. One option is to document time as you would for the Medicare program. The second option is at least document total treatment time.In addition, your respective state practice act may also dictate what you may need to document in terms of treatment time. Make sure you are compliant with both insurance company regulations and your state practice act.
