Don't leave any stone unturned when it comes to receiving the bonus

Refer to this handy chart for information on which quality measures you can report and which codes should go with them.

Important: You must indicate you performed a measure with the appropriate "quality data code" provided by CMS. But you cannot report a quality data code unless your claim also has a relevant CPT code (and an appropriate ICD-9 code if your quality measure deals with diabetes care). Also, to qualify for the 1.5 percent bonus, you must perform the quality measure on 80 percent of the patient population indicated below in conjunction with the measure on at least three measures.
