Processing Special Reports

Question: Which code should we use for filling out forms? Will it be paid? Connecticut Subscriber Answer: Report CPT 99080 (Special reports such as insurance forms, more than the information conveyed in the usual medical communications or standard reporting form). Use this code for filling out school-sports and camp forms.

This code is not likely to be paid. If filling out forms is a noncovered service according to the payer contract, you may bill the parent. However, pediatricians don't usually bill parents for standard medical reporting forms like school-sports and camp forms.
