Question: Our pediatrician reviewed a large medical chart prior to seeing a patient who is in care management with multiple chronic illnesses. Can we bill for this using 99090 in addition to 99487? Florida Subscriber Answer: CPT® guidelines bundle numerous services into the care management service codes. So, if you are billing 99487 (Complex chronic care management services ... 60 minutes of clinical staff time ... per calendar month), you cannot also bill for 99090 (Analysis of clinical data stored in computers (eg, ECGs, blood pressures, hematologic data)), as this and similar services such as 99091 (Collection and interpretation of physiologic data …requiring a minimum of 30 minutes of time) are considered a part of chronic and complex chronic care management. As the chronic and complex chronic care management codes are all time based, you could use the add-on code +99489 (... each additional 30 minutes) if the amount of time your provider spent reviewing the chart and seeing the patient lasted for more than 75 minutes.
